A focused education visit led by a Registered Nurse using plain-language materials and the teach-back method to confirm understanding. We help you learn safe self-care, practice skills, and build a simple routine you can repeat on your own.
Scope: RN education and skill reinforcement only—no diagnosis or prescribing.

Brief intake to confirm goals, current routine, and barriers at home; then build a realistic daily routine you can repeat.
Medication & Device Education (education only—no changes to orders):
Step-by-step teaching for medications and home devices (e.g., BP cuff, glucometer, inhalers). Focus on safe use, timing/routines, and what to monitor—no changes to orders.
Teach-Back Skill Practice
Hands-on practice using teach-back: RN demonstrates, then you (and/or caregiver) practice until steps are safe, consistent, and comfortable.
Take-Home Tools & Next Steps
Simple checklists/trackers and a Next Steps guide so you know what to do at home and what questions to bring to your next appointment (if needed).
Safety guidance (always)
We’ll review urgent warning signs (e.g., severe shortness of breath, chest pain, confusion, very high/low readings, signs of infection) and exactly who to call and when—including after-hours options.

Do you bring equipment?
We teach using your devices to match real-life conditions. We can recommend options to discuss with your provider or pharmacist.
Can you change my medication plan?
No. We educate and reinforce skills. Any changes must come from your ordering provider.
Do you offer follow-ups or virtual refreshers?
Yes—many clients schedule a brief follow-up to confirm technique or add a new topic.
Is this covered by insurance?
Coverage varies. We can provide a visit summary you may submit to your plan or HSA/FSA.
