Support for diabetes, hypertension, COPD, and heart-failure self-management: symptom tracking, SMART goals, action plans, and care navigation to help you follow your plan safely at home.
About this
A focused education visit led by a Registered Nurse using plain-language materials and the teach-back method to confirm understanding. We help you learn safe self-care, practice skills, and build a simple routine you can repeat on your own.
Scope: RN education and skill reinforcement only—no diagnosis or prescribing.
Personal Action Plan
A simple, realistic plan with weekly goals, daily habits, and tracking that fits your life (not an overwhelming program).
Medication Literacy (education only—no changes to orders)
Education to support safe follow-through: what meds are for, timing/routines, and common safety considerations—no medication changes.
Symptom & Vitals Tracking
Support with monitoring tools (BP cuff, glucometer, pulse ox, weight logs) and trend awareness—what to track and what changes matter.
Lifestyle Skills & Routines
Practical coaching for food basics, movement, pacing/energy conservation, sleep routines, and stress tools—small steps you can repeat.
Safety guidance (always)
We’ll review urgent warning signs (e.g., severe shortness of breath, chest pain, confusion, very high/low readings, signs of infection) and exactly who to call and when—including after-hours options.
How the visit works
Assess & align: We clarify your goals, review meds and devices, and note any safety concerns at home.
Teach & practice: We break tasks into small steps, demonstrate, and have you teach back to confirm understanding.
Make it stick: We customize checklists, label supplies, and build a realistic daily routine.
Wrap-up: red-flag guidance and a Next Steps Guide.
Note: RN services complement—not replace—medical care. Clients remain responsible for maintaining ongoing medical care and follow-up as needed.
