About this

A focused, in-home visit where a Registered Nurse completes a wound assessment with measurements and documentation, performs ordered dressing changes, and teaches off-loading and hygiene to support healing and prevent complications. We document every step for a clear record and provide guidance on when follow-up is needed.

Scope: Nursing interventions under active provider orders and education; no diagnosis or prescribing.


 

Wound Status Review

RN assessment of current wound condition: measurements, tissue/exudate review, periwound and limb status, pain level, and current dressing routine.

Current Dressings & Daily Care Routines

Review and discussion of what’s being used and how: supplies, dressings/securement, change frequency, off-loading devices, and storage/sanitation practices.

Patient & Caregiver Teaching

Hands-on education covering Proper hygiene and safe wound handling, Off-loading techniques to reduce pressure, Prevention strategies to minimize complications.

What’s included

  • Structured RN assessment: measurements, exudate/odor/edge/periwound review, photo capture if your provider/policy allows.
  • Dressing changes per orders: cleanse/prepare, apply ordered products, secure/label, and dispose safely—following clean technique and infection-control best practices.

  • Off-loading & hygiene teaching: pressure relief setup, repositioning schedules, moisture management, and clean workflow in your home.

  • Pain/symptom check & safety cues: what to watch for and when to escalate.

  • Documentation & Next Steps: documentation of care provided and clear guidance on what changes to monitor and when to seek urgent care. Clients should contact their provider if changes occur.

How the visit works

  1. Verify orders & goals: confirm written orders (location, dressing type, frequency, cleansing solution, adjuncts) and your questions.

  2. Assess & measure: objective measurements and description, with comparison to prior notes when available.

  3. Perform ordered care: dressing change and securement exactly as prescribed; reinforce positioning/off-loading.

  4. Teach & reinforce: simple, step-by-step guidance you or a caregiver can repeat between visits.

  5. Document & Next Steps: chart the visit and review what to monitor, what’s urgent, and what to do next if changes occur. Clients should contact their provider if changes occur
