Diabetes Essentials
Clear guidance on A1C/targets, reading glucose patterns, hypo/hyper warning signs, meal timing, label reading, activity pacing, and sick-day basics.
Healthy Aging Toolkit
Home safety setup, balance/strength basics, hydration and nutrition cues, sleep hygiene, brain-friendly routines, and prompts for advance planning.
Medication Literacy, Skills & Take-Home Plan
What each med does, when/how to take it, food/alcohol interactions, missed-dose plan, and safe storage/disposal—plus hands-on BP/glucose technique using your devices, tracking logs, and take-home materials (checklists, daily plan, symptom/reading logs, and a provider discussion list).

No. We do not diagnose conditions or prescribe medications.
Will you change my treatment plan or device settings?
No. We don’t alter your plan or adjust device settings. Any changes must come from your ordering provider.
Does this replace my provider’s advice?
No. Education complements—not replaces—your clinician’s guidance.

Safety & Escalation
What warning signs will we cover?
We review urgent red flags—severe low/high blood glucose, chest pain, sudden shortness of breath, new confusion, allergic reactions, and medication side effects—and exactly who to call and when (provider, pharmacy, emergency services). We also encourage prompt follow-up with your ordering clinician for any changes.
Session Logistics
How long is the session?
Most visits run 60–90 minutes, depending on topics and practice time.
Do you bring equipment?
We teach using your devices for real-life accuracy. We can suggest options to discuss with your provider or pharmacist.
Can a caregiver join?
Absolutely—caregivers are welcome and encouraged.
Is this covered by insurance?
Coverage varies. We can provide a summary for your records or HSA/FSA.
Follow-Up & Ongoing Support
Do you offer follow-ups or classes?
Yes. We can schedule brief refreshers or small-group workshops for ongoing support.
