Patient & Caregiver Teaching
Hands-on education covering Proper hygiene and safe wound handling, Off-loading techniques to reduce pressure, Prevention strategies to minimize complications.
Action List for Provider Discussion
A structured list of items to review with your physician, NP, or PA.
Safety guidance (always)
We’ll review urgent warning signs (e.g., severe shortness of breath, chest pain, confusion, very high/low readings, signs of infection) and exactly who to call and when—including after-hours options.
How the visit works
Assess & align: We clarify your goals, review meds and devices, and note any safety concerns at home.
Teach & practice: We break tasks into small steps, demonstrate, and have you teach back to confirm understanding.
Make it stick: We customize checklists, label supplies, and build a realistic daily routine.
Wrap-up: You’ll get your take-home materials, red-flag guidance, and a provider action list.
Note: RN services complement—not replace—medical care. We collaborate with your physician/NP/PA and follow provider orders and facility policies.
