What this service is
A focused, in-home visit where a Registered Nurse completes a wound assessment with measurements and documentation, performs ordered dressing changes, and teaches off-loading and hygiene to support healing and prevent complications. We promptly communicate concerns to the ordering provider and document every step for a clear record.
Scope: Nursing interventions under active provider orders + education; no diagnosis or prescribing.
Wound Status Review
RN assessment of current wound condition: measurements, tissue/exudate review, periwound and limb status, pain level, and current dressing routine.
Current Dressings & Daily Care Routines
Review and discussion of what’s being used and how: supplies, dressings/securement, change frequency, off-loading devices, and storage/sanitation practices.
Patient & Caregiver Teaching
Hands-on education covering Proper hygiene and safe wound handling, Off-loading techniques to reduce pressure, Prevention strategies to minimize complications.

How the visit works
Verify orders & goals: confirm written orders (location, dressing type, frequency, cleansing solution, adjuncts) and your questions.
Assess & measure: objective measurements and description, with comparison to prior notes when available.
Perform ordered care: dressing change and securement exactly as prescribed; reinforce positioning/off-loading.
Teach & reinforce: simple, step-by-step guidance you or a caregiver can repeat between visits.
Document & communicate: chart the visit and contact your provider promptly if red flags or order clarifications are needed.

Safety & escalation (always)
We’ll review red flags—fever, rapidly spreading redness, foul odor or markedly increased drainage, uncontrolled bleeding, severe or rising pain, or signs of systemic illness—and outline exactly who to call and when. We also notify your provider promptly if assessment findings suggest a change may be needed.

Do I need provider orders?
Yes. All wound interventions are performed under active orders.
What if I don’t have orders yet?
We can provide an education-only consultation and a clear action list to discuss with your provider.
Do you bring supplies?
We use your ordered supplies and can suggest options for your provider to prescribe; we don’t dispense prescription items.
How long is a visit?
Typically 45–90 minutes, depending on number of wounds and teaching needs.
Will you update my provider?
With your consent, we share relevant findings and concerns promptly and request clarifications when needed.
